Appointments/Office Hours/Location APPOINTMENTS Patients are seen by appointments only. Please call in advance so that we may reserve a time for you. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. If possible, please give us at least 24 hours notice. This courtesy on your part makes it possible for us to give your appointment to another patient. New Patient Health History Form EMERGENCY CARE We reserve a limited amount of time each day in order to quickly care for our patients with emergency needs. If you have a problem requiring treatment, please call us as early in the day as possible. Patients of record with emergencies after hours can contact Dr. Moore at home.
